Notice: You will be aware that the practice website looks a little different.  This is because we have merged the websites of Newmarket Road Surgery and St Stephen’s Gate Medical Practice together to represent the one partnership we now are, operating over the two sites.  The new  website should allow you to find all the information you need easily and clearly.  All the online services such as booking appointments and ordering repeat prescriptions operate in exactly the same way as before.  If you notice anything is missing or not working correctly, especially those patients who were used to the old Newmarket Road Surgery website, do contact Rosemary Moore the practice manager who will investigate.

We want to know if you need information in braille, large print or easy read. We want to know if you need a British Sign Language interpreter or advocate.

Please note, this form should be completed if there are children in the family under 5 years old. Once again please ensure this form is filled in full with as much information as you can remember.

We need to have up to date contact information for all our patients. This enables us to make contact quickly and effectively.
